Financial Policy & Payment Options

We are pleased to be an active member of your healthcare team. We will bill your insurance carrier(s) according to the information given by you at registration. If for some reason, you are in doubt about your insurance information, you will be responsible for the bill. It is a good idea to carry your insurance card(s) and photo ID with you.

Suburban Lung Associates, Chicago Sleep Group and Chicago Chest Center accept assignment of benefits and maintain an active follow up program with all health insurance carriers. If your account remains unpaid after 60 days, please contact your insurance carrier for payment status. Ultimately, it is your responsibility to pay for services.

Payment Options

Co-pays and/or deductibles are due at time of service. Any balances left after your insurance company pays are your responsibility. When making a payment you have the following options:

  • Cash
  • Check
  • Money Order
  • Mastercard, Visa, Discover

A late fee will be applied to accounts for non-payment of a copay due at the time of service.

Payment Plans

Account balances must be paid in full within 30 days of receipt of balance due statement. To assist in resolving your outstanding balance, monthly payment plans are available for balances greater than $500. Payment plans generally consist of 3 equal monthly payments. State of Illinois Public Aid

This program is available through the State of Illinois and the Illinois Department of Public Aid. Eligibility is based on state criteria. If you would like additional information, contact your local Public aid office.

Hospital-Related Financial Assistance

Most of our hospital partners are concerned about the financial burden that results from medical bills. As a result, you may qualify for full or partial reduction in your medical bills. Eligibility is based on yearly gross income. Please ask to speak to a financial counselor at the hospital for hospital-related bills.

Payment Philosophy

We are sensitive to the financial needs of each of our patients. We participate in most HMOs, PPOs, and other insurance plans including Medicare and Medicaid. Our Billing Service is available to work with you on an individual basis to resolve your account. We appreciate the opportunity to care for you and your family.
